Step 1: Contact Us
Call us at 1-888-EMPOWER, email us, or walk-in and start your evaluation process! We will answer any questions you have and describe the evaluation process to you. We will also explain if you’ll need a medical record documenting your qualifying condition. There are many conditions that qualify for the medical marijuana program in Oregon. Any condition that causes pain, nausea, or muscle spasm can qualify with a record of treatment. Some medical conditions like cancer, HIV, multiple sclerosis, epilepsy, glaucoma, and PTSD require very little documentation. If you have previously seen another doctor for your condition, we will obtain the record for you after you sign a medical release form.
Step 2: Schedule Appointment
Once you are ready to schedule your appointment, we will find a convenient appointment time that works well for your schedule.
Step 3: Meet the Doctor
When you come in for your appointment, our staff will guide you through the paperwork and introduce you to the doctor. The physician will perform a medical evaluation, review your medical history, describe the proper use of medical marijuana and answer any questions you have. If you have a qualifying condition, the doctor will complete the physician form required by the state, necessary paperwork for the following two steps.
Step 4: Same Day Submission
Immediately after your appointment, we will help you upload your application to Oregon’s Medical Marijuana Program.
Step 5: Get Your Card
Once you have submitted a complete application, you can print the 30-day receipt, which has the same legal effect as a registry ID card for 30 days. Shortly after your application is completed, the state mails your registration card directly to you!
